How to Use the Medication List Feature?

Learn how to manage and sync medications and allergies across your client records and notes.

The Medication List tab, available on both the patient page and the note page, is designed to give you one clear place to manage your patient’s medications and allergies.

Instead of updating this information in multiple places, everything lives here and stays aligned with your notes. When you update a medication or allergy in the tab, it’s reflected in your notes.

You can also let Berries do the work for you. By using Complete Note, your Medication List can be automatically updated based on what was documented in the session. And if something new is mentioned in a note, it can be surfaced here for your review.

1. Setting up your list

When you open the Medication List for the first time, you’ll start by creating your patient’s baseline. Getting set up is simple, and you have a few options depending on what you already have.

You can:

  • Paste an existing list and let Berries structure it.

  • Import from past notes if medications or allergies were already mentioned.

  • Add everything manually if you prefer to start fresh.

2. Understanding medication statuses

Each medication includes a status to help you understand how it’s being used:

  • Main statuses:

    • Active - currently taken on a regular basis.

    • Titrating - the dose is currently being adjusted.

    • Discontinued - no longer taken, but kept for history.

  • Additional status:

    • PRN - taken as needed (could appear alongside any main status).

Once your list is set up, you can also add allergies at any time by switching to the Allergies view.

3. Adding and managing medications and allergies

You can update your Medication List at any time as things change.

For medications, you can:

  • Add a new medication

  • Edit details like dose, frequency, or notes

  • Update the status (Active, Titrating, Discontinued, with PRN if needed)

  • Mark as Entered in Error to remove it permanently

For allergies, you can:

  • Add a new allergy

  • Delete it if needed

To see more details, click Details on any medication. This opens the full record, including a history of changes. You can also click on any history entry to view the previous version.

4. How it connects to your notes

One of the most helpful parts of the Medication List is how it stays in sync with your notes.

There are two ways your list can be updated after a session:

1. Review suggestions manually

After a note is generated, suggested updates appear in the Medication List tab.

Click Review updates and choose what to apply.

2. Complete the note

Completing a note updates the Medication List automatically.

You can see what changed by clicking View updates.
